Tras revisar los metaanálisis de las pruebas de productos homeopáticos de los últimos 20 años, el fisiólogo y homeópata británico concluía que, “a pesar del importante incremento de la actividad investigadora [en homeopatía] desde 1994, la preocupación por la calidad de los estudios limita la interpretación de los datos de los ensayos controlados aleatoriazados disponibles”, ya que las investigaciones han sido habitualmente de “baja calidad”.

“Lo importante de este articulo, en mi opinión, es que un homeópata de la Asociacion Homeopática Británica se atreve a decir en una revista como Homeopathy lo que todos llevamos años diciendo, que no se pueden extraer conclusiones sobre la eficacia de los productos homeopáticos en base a los ensayos clínicos realizados con ellos dada la baja calidad científica de los mismos, y por tanto, a día de hoy, no existe ninguna evidencia científica de que el efecto de la homeopatía vaya mas allá del placebo”, destacó Koldo Callado, profesor de Farmacología de la Universidad del País Vasco (UPV), en su intervención en el último encuentro Enigmas y Birras de Bilbao.

Homeopathy, la principal revista con revisión por pares dedicada a esa práctica, ha perdido hace poco su índice de impacto “por un excesivo número de autocitas (el 71% de sus citas lo son)” y no aparece en el Journal Citation Reports 2015, explicaba hace unas semanas el físico Francisco Villatoro. Fundada en 1911 como el British Homoeopathic Journal, la publica en la actualidad Elsevier.

Una molécula en varios universos

Los principios de la homeopatía, establecidos por el médico alemán Samuel Hahnemann en 1796, son que una sustancia que provoca un síntoma puede curarlo si se diluye mucho y que, cuanto más diluida esté, mayores serán sus efectos sanadores. Hay de venta en farmacias preparados homeopáticos cuyo principio activo está diluido en el equivalente a un tercio de gota de agua en todos los océanos de la Tierra y hasta a una molécula en varios universos. Pruebas de laboratorio hechas en todo el mundo han demostrado que esos preparados homeopáticos no tienen más que azúcar. Muy caro, pero sólo azúcar. “Los principios de la homeopatía son tonterías”, sentenciaba en mayo en un vídeo la Sociedad Estadounidense de Química (ACS), que cuenta con más de 158.000 miembros

En marzo, tras la suspensión del máster en homeopatía de la Universidad de Barcelona porque no hay pruebas de la efectividad de esa práctica, Valérie Poinsot, directora general delegada del Grupo Boiron, la principal multinacional del sector, aseguró, contra más de 200 años de estudios, que sus productos funcionan, aunque ellos no saben por qué. “Los pacientes no necesitan la evidencia científica de un medicamento, sólo que funcione”, dijo la alta ejecutiva de la firma francesa, que añadió, supuestamente en defensa de su negocio, que también “mucha gente critica la Nutella, pero a los niños les gusta”.

Abstract
INTRODUCTION:
Observations about controlled clinical trials expressed by Max Haidvogl in the book Ultra High Dilution (1994) have been appraised from a perspective two decades later. The present commentary briefly examines changes in homeopathy research evidence since 1994 as regards: the published number of randomised controlled trials (RCTs), the use of individualised homeopathic intervention, the 'proven efficacy of homeopathy', and the quality of the evidence.

METHODS:
The commentary reflects the details of RCTs that are available in a recently published literature review and by scrutiny of systematic reviews of RCTs in homeopathy.

RESULTS:
The homeopathy RCT literature grew by 309 records in the 18 years that immediately followed Haidvogl's article, with more than a doubling of the proportion that investigated individualised homeopathy. Discounting one prior publication, the entire systematic review literature on homeopathy RCTs post-dates 1994. A total of 36 condition-specific systematic reviews have been identified in the peer-reviewed literature: 16 of them reported positive, or tentatively positive, conclusions about homeopathy's clinical effectiveness; the other 20 were negative or non-conclusive. Reviews typically have been restricted in the strength of their conclusions by the low quality of the original RCT evidence. Three comprehensive systematic reviews concluded, cautiously, that homeopathy may differ from placebo; a fourth such review reached negative conclusions. A recent high-quality meta-analysis concluded that medicines prescribed in individualised homeopathic treatment may have small, specific, effects.

CONCLUSIONS:
Despite important growth in research activity since 1994, concerns about study quality limit the interpretation of available RCT data. The question whether homeopathic intervention differs from placebo awaits decisive answer.
